Information is the lifeblood of the health care delivery system. The medical record, in manual or automated form, contains the medical information that describes all aspects of your patient care at Permian Regional Medical Center.It is a communication link among caregivers, as well as providing data for medical research, peer review, public health studies, and substantiating reimbursement.
Mission Statement
The mission of the Health Information Management Department of Permian Regional Medical Center is to provide quality transcription, complete retrievable and accurate documentation for research, and a guide for treatment.Birth Certificates
Our department works cooperatively with Texas Vital Statistics to insure that accurate information is submitted on each child born at Permian Regional Medical Center. Our electronic birth certificate program takes 1 day after submission to reach the Texas Vital Statistics then an additional week for the Country Clerk's office to retrieve it for their records and/or for patient copies.Legal Forms
Copies of any and all legal forms given to PRMC Medical Records will be scanned into the patient's electronic medical record to ensure that the appropriate staff has access to the documents to carry out the patient's wishes that they have requested. Examples of legal forms include but are not limited to: Advance Directives, Power of Attorney, Do Not Resuscitate (in and out of the hospital), Living Will and court papers.If you have questions whether a document is considered a legal form, please call the number below.
Texas Cancer Registry
As mandated by the Texas Cancer Incidence Reporting Act and the Texas Department of Health, in cooperation with the Center for Disease Control (CDC), Permian Regional Medical Center is an active participant with the Texas Cancer Registry doing our part for cancer research.Your Medical Record
Every effort is extended to protect the information contained in a medical record of all patients of Permian Regional Medical Center. The Health Information Management Department is a secured area. Confidentiality is a priority that does not waiver.All records are retained for the period of time specified by the State of Texas, with the retention period of 10 years for adults and to the age of 21 years for minors. Copies of your medical records can be obtained for a nominal fee.
Requesting Medical Records
Permian Regional Medical Center maintains medical records for each patient treated at the hospital. To request your medical records, please visit our Health Information Management department between the hours of 8:00 AM - 5:00 PM, Monday through Friday, or call 432-464-2121. You may also print and fill out a Medical Record Request form in the link located under Medical Record Request.Prior to receiving copies of records or authorizing records to be disclosed, you will be asked to complete the appropriate form and provide proof of your identification with a valid, government issued photo ID.
The appropriate form, along with a copy of your proof of identification, may be faxed to 432-523-3605.
Requests for medical records could take up to 72 hours to process. There could be a charge for copies of the medical records and copies of Radiology disk.

Records needed for continuing care purposes do not require the completion of the above reference form, but rather will be sent directly to your provider. (Unless if you need records sent to another physician that did not order the tests/treatment.)
